Lord, my Creator and Redeemer, I wholeheartedly accept the sentence of my death, according to Your will and in a spirit of adoration.
I intend to die as a devout member of the Church and to go into eternity with the best dispositions of faith, hope, charity, and sorrow for my sins.
I hope to renew, at least mentally, the baptismal promises and religious vows.
I offer You, Lord, all the circumstances, even the most painful, which will accompany my passage to eternity, in reparation for my sins and to merit heaven better.
I invoke the great patrons of a happy death from whom I hope to receive merciful assistance: Jesus Crucified, with whom I intend to pronouce the words: "Father, into Your hands I commend my spirit"; Mother Mary, that she may pray for "us sinners now and at the hour of our deat"; and St. Joseph, that he may obtain for me a holy life so as to merit a death similar to his.
Agonizing Jesus, sorrowful Mary, St. Joseph, I ask You for these graces:
A good life - in faithful observance of the commandments and all of the duties of my state in life - which assures me of a holy death; on this my eternity depends.
The gift to receive, in the event of a serious sickness, the sacraments of Reconciliation, the Anointing of the Sick, Viaticum; and to obtain the plenary of indulgence.
Correspondence to my vocation, according to the gifts I have received, that my life may realize the utmost good, for the glory of God and my eternal happiness.
Daily prayer, needed by everyone for salvation; in particular the frequent celebration of the sacraments of Penance and Holy Eucharist; devout participation in the Liturgy.
Jesus Master, I believe.
Jesus Master, I hope.
Jesus Master, I love You.
Jesus Master, I invoke Your mercy.
Jesus, Mary, and Joseph, I give You my heart and my soul.
Jesus, Mary, and Joseph, assist me in the hour of my death.
Jesus, Mary, and Joseph, may I die and rest in peace with you.
